[Name] CHECKBOX

[Description] This creates a checkbox with a label. Depending on the state of
              the checkbox, checkedText, or uncheckedText is returned.
              Note that quoting can be used to have values with spaces (see second example).
              
              This type will normally appear with the label to the left.  If
              you want the label to the right, use CHECKBOX_WITHLABEL


In a Transformer (*.fmx) File

   Syntax

      PARAMETER_NAME: <macroName>
      PARAMETER_TYPE: CHECKBOX <checkedText>%<uncheckedText>
      PARAMETER_PROMPT: <label>
      PARAMETER_DEFAULT: [<defaultValue>]
   
   Example

      PARAMETER_NAME: IS_CHECKED
      PARAMETER_TYPE: CHECKBOX yes%no
